The Division of Information Technology has streamlined the procurement process for IT products and services to better assist you. This intake form will help capture important information needed to serve you effectively and efficiently.

Helpful Hints Before You Begin

• You will be able to save your progress and return to the form later if needed. You must use the same Internet browser and have browser cookies enabled in order to return to your location in the form.
• You will be asked to upload documents if you have them. They include:
  • vendor quotes/bids/statement of work/terms of service
  • VPAT to demonstrate electronic accessibility (what's a VPAT?)
  • URL to vendor's accessibility information if VPAT not available
  • HECVAT (what's a HECVAT?)
  • Penetration test (pen test) results (what's a pen test?)
  • TX-RAMP Certification (what's TX-RAMP?)
• You may be asked for additional documentation from the Information Security Office, such as the HECVAT or penetration (pen) test results. You'll need to consult with your vendor for those pieces.
• We understand you may be entering this request on behalf of someone else. We need contact information for all parties involved (person completing the form and the person using the product) if there are additional questions.
• You will receive a copy of this form with your responses for your records. The appropriate area will follow-up with you upon completion of this form.
